Some of it is about speed and circumstances.
1. When 6 AS were trying to capture a false dragon, they wanted to shield him. He on the other hand didn't need to try to shield the women. Just kill them faster than they could capture him. Once you kill 2 or 3, then you can shield the other 3.
2. Same goes for Rand. Even when the circle of 13 took him in LOC, he had a few quick moments of warning. Had he lashed out with the power and killed the women standing in front of him he would have easily escaped (specially since the woman in front of him was leading the circle, so killing her would have given him more time). So the element of surprise is essential in the fight.
