We assume that "he" is Rand, but maybe it's another man, e.g. Logain or Narishma (Rand has only one hand, after all).
Furthermore, "a blade of light" might also mean Justice, though Callandor seems naturally more obvious.
Finally, who or what "the three" are, why they "shall be one" and if "he" is one of the three or an additional person besides the three (like in the "three in a boat and the one who is dead yet lives"-Foretelling) is unclear.
Many people also seem to assume that this Prophecy is most crucial, likely the key to defeat the DO in the end, although it seems that Min just picked one random Prophecy to interpret. Plus, the other Prophecies we positively know dealing with Rand's final confrontation vs. the DO imply a duel between only Rand & the DO and the importance of Rand's blood. This indicates that this Prophecy will happen before Rand faces the DO.
Having said that, my guess is that this Prophecy indeed refers to Rand, his three girls and either Callandor or Justice.
Probably it's plot-wise linked to Nicola's Foretelling: "The lion sword, the dedicated spear, she who sees beyond. Three on the boat, and he who is dead yet lives. The great battle done, but the world not done with battle. The land divided by the return, and the guardians balance out the servants. The future teeters on the edge of a blade", because this Foretelling also mentions Rand, his three girls and a blade.
Possibly it's furthermore plot-wise linked to Min's Viewing of "three woman standing over a funeral bier with him on it" and the Wise Ones Dream "of you [Rand] on a boat with three women whose faces they could not see and a scale tilting first one way and then the other", because we see in all these Prophecies again Rand with his three girls in an apparently crucial situation.
We didn't yet learn that this important scene is prophesied in the Karaethon Cycle, too. But it makes sense that it's also mentioned there, by stating another detail, namely that Rand will hold a blade of light accompanied by his girls for some reason.
