I remember catching quite a few, but none of them were very disruptive, and they did have to rush the book a bit. Probably get removed for the paperback. I remember in Roger Zelazny's amber series there was a piece where the hero refers to 'my mother Jasra' when that's actually his enemy's mother, and even in the audiobook read by the author, years after publication, Roger Zelazny repeated the phrase.
