This is why Nynaeve found Rand's short-hop trick "clever." Say A is starting point, B is close to A, and C is the destination (far away). If one doesn't know A well, one can Travel from A to B (short distances for Traveling are easy). The act of Traveling to a location makes that location well enough known to travel from it. As a result, after Traveling to B, you can Travel anywhere you'd like (i.e. C).
I don't know how many days have passed since Nynaeve took Lan to Land's End, but it can't be that long ago, time-wise. Anyway, Nynaeve used this very "trick" to drop him off and then go round up recruits. In fact, there was a reply from RJ himself in his blog, because so many people had asked about it:
"For several people. Nynaeve could Travel after depositing Lan in Saldaea because she had “learned” that spot by Traveling to it. Remember, if someone Travels to a place, they now know the place they have Traveled to as well as if they had spent time there learning it." Dec. 2005 blog entry
So, either Sanderson messed up, or, Nynaeve doesn't know squat about Travelling, or, she's been smoking some of her "herbs".
And this is a very, very obvious one. First off, as you pointed out Nyn already did this....she knows this. Also, if we readers...even as outsiders but never having actually used the OP, so to speak...can deduce the obviousness of this situation, how could it possibly be such a surprise to experienced OP users?
This is where the editor/assistants come in. They failed pretty badly in some areas, this being one of them. Some very basic situations like this were messed up, and they shouldn't have been. Somebody should have caught this.
I would slave, to pave the way, to sink your ship of fools.
I hate that Mat...

Traveling requires you know your origin well, not your destination

I can't understand how such mistakes can happen

Well, let's be clear here - Nynaeve didn't "know" - she's not the best or most thoughtful thinker
