I don't think it's the wards that are failing, so much as the strength of decay and pestilence is increasing as the DO grows stronger, overcoming the normally effective wards.

And the text doesn't make any mention of Egwene being wary or cautious about picking up the wand, she just runs up and grabs it. You'd think she would be more careful.
