I read an interview where - at least as I understand it - BS openly stated that the ending of TGS truly meant that Rand had indeed become mad due to his stress (not the Taint) and that LTT's voice was just a delusion. IIRC, this would mean that Rand was already mad in TSR or TFoH, because he started to hear LTT's voice then. Maybe BS could verify this to you, so that we have a 100 % certainty on this matter.
It would seem Rand and Lews Therin were always one and the same. He had the memories of both men, but his mind split the other man into a separate voice that he would communicate with. At the end of TGS he realizes the voice was never a separate entity and he's just been talking to "himself" the whole time.
