I think Graendal may be alive still... And here is my reasoning, bear with me, I just finished with the book and will reread for details soon.
1. Balefire to my knowledge burns a thread out of the pattern farther back with the more power you pump into it. We know that this is true of people, but what about objects? What if you trip on a rock and break your leg and then turn around and balefire the rock out of existance so you wouldnt have tripped on it and would not have broken your leg. Makes sense right?
2. Rand presumably put more power into that balefire than absolutely necessary, my guess with using the access key, he put enough power into that blast to knock that house back a few hundred years. Now if the house ceased to exist some unknown time frame past (again, the size of the blast would make one assume a massive amount of power was put into it), then why would graendal have been there to have her base of power? I understand that if she was caught in the blast, then she would be gone no matter what, but I will assume that she was not home for the blast.
3. We are relying on a compulsion that was placed on rands man to determine whether or not she was killed, she could have placed it on him to lift after he told his story but I dont believe it so. When asked after the balefire, he doesnt seem to remember going to the house.
I believe that rand blasted the house back to the AOL, his man does not remember or have the compulsion upon him because there was no house to go into to talk to graendal, and that graendal escaped to somewhere just before the blast hit and is not lying low.
I know that there are some holes there, but I think that killing was just too easy...

Methinks another channeler (Darkfriend/Black Ajah) placed the Compulsion on Rand's bait.

Two reasons: The level of Compulsion, and Forsaken paranoia.
