Most likely, the Eelfinn killed her by draining her OP capacity too quickly.
This is what they told Moiraine. Someone came for her. Moridin. But it's possible she was dead when he came for her, because I have no explanation for her body change, unless it was a wish.
To be able to attract Rand again. This could have also been her wish. A new body, a new look, maybe even more beautiful in a different way. More like...Ilyena?
Rand seemed to be somewhat interested in her when he saw her..
It makes no sense that her hearts desire in WH and TPoD seems to be killing Rand (Moridin won't even let her near him for fear she'll kill him).
Yeah it does not make any sense, however since Lanfear went thru the doorframe ter'angreal it appears that she was allowed the 3 requests as was Moiraine. Some wish regarding Rand seems likely for her character. I suppose having him killed wasnt a request they would grant
