We know that balefire doesn't destroy your soul, but that sort of conclusion isn't present with other murderous things we've seen in the series. State why you think the following do/do not destroy the soul permanently...
1. Being kissed by a draghkar
2. Being used as a vassal to forge a Myrddraal blade
3. If your mindtrap is destroyed.
4. If the DO wants to destroy your soul (assuming you're in the Pit of Doom)
5. Death by mashadar
6. Others you would like to suggest
The Myrdraal blade would seem to trap your soul for a time, but Demandred said that it needs to be replenished after a while, so I would assume that the soul then escapes and must be replaced by a new one.
Mashadar would trap your soul until such time as Mashadar is destroyed at the end of the age or another age - but at some point Mashadar is destroyed before being created again in the next turning of the Wheel.
The Draghkar's kiss, mindtrap destruction etc won't destroy your soul either, I don't think.
Being trapped in a vacuole might imprison your soul out of time, and therefore technically keep you from being reborn for eternity, so that might be a form of destruction.
No idea about the Pit of Doom.
