My first inclination was that it belonged to an incarnation of the dragon between Lews Therin and Rand Al'Thor. This dragon would have been thought to be a false dragon.
If the theories are combined then perhaps it is the sword of Guaire Amalasan who shared the same soul as Lews Therin and Rand Al'thor.
This is sort of ruled out by the KC. Rand is the direct rebirth of LTT, there's no one in between. The Heroes at Falme would have known and by their customs they would have called Rand by the name of this last incarnation - Jordan's own explanation. They called him LTT. There's no in-between incarnation.
