How did you read the ending? I think they broke through his wall for good and he was aware of what his wife did and that he killed her. I think he only pretended to go back into the loop, because he preferred the lobotomy over the knowledge.
It seemed obvious to me when he said "Is it better to live as a monster or die as a good man?" before he went off with the doctors. As in, the only choices for him were to accept that he was a "monster" and live in reality or continue with his delusions that he was a good man and "die" by being lobotomized. He chose the latter because he couldn't live as a "monster."
Btw, did anyone find it hilarious that one of the doctors was carrying the lobotomy instruments with him when they came for him? That's like going to the dentist and having him come out to meet you with a drill in his hand
