According to various sources, it will be set in Skyrim and will be pretty similar to ES4, probably using the same engine or an upgraded version of the same. There are also rumours that a setting-appropriate version of VATS will be included after its success in FO3, although I suspect that's more wishful thinking at this stage.
Since they outsourced the FALLOUT 3 'proper' expansion (NEW VEGAS) to Obsidian, who are now making it as a full, stand-alone game, the assumption is that they've had people working on SKYRIM at a preliminary stage since FO3 came out and full work got underway on it after MOTHERSHIP ZETA was completed. My guess is that we'll get an official announcement at E3 next year, if not sooner.
As someone else said, it is unlikely that an ELDER SCROLLS MMORPG will be developed at the same time as the FALLOUT one they have a partner studio working on.

The reason Elder Scrolls is good is because it isn't multiplayer. MMORPGs suck. *NM*
