1. Do you have misgivings about getting into steel cans with wings and hurtling miles above the earth?
Yes. I keep waiting for gravity to notice. Or, you know, for the plane to just snap in two like an egg and scatter us .... I am probably not helping.
2. If you do, how do you distract yourself during the particularly horrible time called "takeoff?"
I drink wine beforehand, which makes me more relaxed, and then I break my boyfriend's arm as we take off. I then drink more wine and read a book and pretend I am on a bus. This works quite well, as I am rather gullible, except when we take a sudden turn, and then I hurt my man some more.
2a. Is takeoff not your least favorite time?
Yes. It is horrible. At least when we land we are going in a controlled fashion in the right direction.
3. Provide hugs and reassurance that this voyage, like every single one I've taken before, will not end in horrible disaster.
Well, I keep being told, by people who should know, that the physics of flying is sound, and that if they lose motor power when taking off (which is what I always think will happen), it will not just slow and then fall back downwards (in the cartoonish way I see in my brain), but begin going forward and be able to land.
Also. I always try to sit on the row behind the wing exit, as that apparently gives a greater chance of survival.
4. Seriously how do you not want to cry when you go into a plane IT IS TERRIFYING
I do, but my pride restrains me. Actually, I want to shout at them to let me out, but I have misgivings about looking like a lunatic as well.

The silly thing is that I fly at least twice a year, and more if you count individual flights.

Recognize that the fear is not a logical one. If tha doesn't help, it's unlikely that anything will.
