Recognize that the fear is not a logical one. If tha doesn't help, it's unlikely that anything will.

1. Do you have misgivings about getting into steel cans with wings and hurtling miles above the earth?
Nope.
1a. WHY NOT?
Because statistically speaking, it's far and away the very safest means of travel. Almost everyone I know has been in a car accident. Thankfully no one close to me has been killed, but I know of a few people who have been, even if I don't know them personally. I think I read that you have something like a 1/100 chance of dying in a car crash over your lifetime, though I haven't checked that stat.
Point is, cars are flippin' dangerous. Getting in a plane is the safest part of your trip.
2. If you do, how do you distract yourself during the particularly horrible time called "takeoff?"
Typically, I spend takeoff trying desperately hard not to stare out the window like a little kid. I've flown dozens of times, but I still enjoy it.
2a. Is takeoff not your least favorite time?
No. I love the feeling of being pushed back in my seat, and watching the ground zoom away. And really, I don't know what's not to like. If you were to look up the stats (I haven't) you'd probably find takeoff to be one of the more accident prone times, but then again, you're on the ground. You're unlikely to die while still on the ground, I'd think. Anyway, takeoff is fun.
2b. What is?
Probably right when they first announce that you're going to land. You start putting away your book/mp3 player or whatever, and then it takes them another 35-40 minutes to actually have you on the ground. Seems to take ages.
3. Provide hugs and reassurance that this voyage, like every single one I've taken before, will not end in horrible disaster.
You'll be fine. I don't see how anyone can reassure you though. Here's what's reassuring: getting on that plane is, most likely, the safest non-trivial activity you will do in the next year or so. If that doesn't help, then your fear is an irrational one.
Good luck! Make yourself do it, don't think about it, and you'll have no problems at all.
4. Seriously how do you not want to cry when you go into a plane IT IS TERRIFYING
No sir it is not. Not only are planes absurdly safe, but if something WERE to go wrong, you'd get one of the fastest most painless deaths I can possibly think of.
...not helping, huh?

The silly thing is that I fly at least twice a year, and more if you count individual flights.

It has to do with the lack of control. (WARNING: I'm not assuring you of anything)
