How long is the cat? We have two cats that are actually very long and the standard litter box you buy at the pet store was useless as they grew to full size. We buy one of those plastic storage totes from Walmart now and just pour the litter in that. The cats have no problem with it and the walls are tall enough to stop the urine from going right out of the box.
A different litter box might help. And definitely take him back to the vet for another once-over.
