I read AGoT and ACoK together in 1999, then ASoS in 2000. Then all three again in 2003, then AFFC on release in 2005, and then all four again at the end of last year and start of this.
So three times for AGoT and ACoK and twice for ASoS and AFFC. I simply haven't had time to re-read any more than that. I've actually read MAGICIAN, the NIGHT'S DAWN TRILOGY, the first six WoT books, LotR and a few Pratchetts and Clarkes far more.
In a way I suppose my position on Westeros.org and constant discussions of the books means rereading the whole thing as a refresher for the memory is unnecessary. If you know who Jon Connington is from memory, you're probably good to go for ADWD without having to reread the series

We sometimes hear about kids doing horrible things

Is that because they may not understand the consequences of their actions?

Most likely, yes. Plus their reasons for doing bad things lack deeper inferior motives.

Arya is old enough to understand cause and effect...

Arya is not a soldier. Soldiers are not individually responsible for the results of war.
