Is there an accurate description somewhere reliable that shows the correct placement and composition of Hemalurgic spikes that are used to create Steel Inquisitors, Koloss, and Kandra? I know the Inquisitors have two spikes through the eyes, but that's it.

You'd have to ask Sanderson himself, or maybe over on his forums, to be completely sure. But as far as I can tell, I don't think it matters. I always thought that the spikes through the eyes (and one on their back, with a few more placed elsewhere) were just to strike fear into the citizens.

Kinda series spoiler, for those that haven't finished it:

Vin had the extra bronze powers because her mom had killed her Seeker sister with the earring Vin wore, then pierced Vin's ear. Spook got pewter powers when a sword killed a Thug and the sword point broke off in his shoulder. From that, I gather it doesn't matter where the metal goes, as long as it pierces the body.
