Because most of the channelers don't seem to be as creative as the legions of readers are. If Elayne or Egwene was there when Rand was captured, I tend to think unlike Nynaeve's initial thought, one of them might have put two and two together, marched to the Guardian and disabled it.
Possibly. But why is it that Rand did not come up with the idea. Surely it would have been more logical to destroy the guardian than to kill thousands of experienced soldiers that could rather be used against the shadow&trollocs.
Still, Rand was already totally crazy there so I might be able to forgive him this stupidity. I would be less willing to do the same for Rand/LTT however so I really hope the new and improved Rand won't be idiotic enough to walk into FM in the next or final book.
Ter'angreal also seem to be very durable, though I'm of the opinion that opening a gateway on it, carving it up with balefire or directing a flow of Fire into it in order to melt it down to slag might have worked.
I would say that unless its made of cuendillar, it can be destroyed. And when we see it, no one even suggests the possibility so I doubt this.
Gateway is the easiest option ofcourse, but balefire or something would work. Balefire has the difficulty of range though. It needs to be something that is channeled from outside the guardian-bubble, but which has a long enough range to affect the guardian itself inside. I don't know if Rand is strong enough to create balefire that can reach the guardian from outside its effect. There are however others ways. Like I said, just lift a huge boulder weighing several tons above the city/guardian and let it crush the thing if nothing else works.

It depends on what you consider to be "the problem"

Yes, there's definitely time for Elayne and Egwene to both be captured at least once
