Graendal is dead. No way could she have perfectly anticipated that situation.

Slayer killed Asmo. Slayer is the one that the shadow uses to kill renegade dark friends. They sent him after Fain. They sent him to kill the two captured BA in the stone of tear. They sent him to kill Asmodean. Asmodean would most likely refer to him as "you!". As in "Oh @%*! you're that guy that we use to kill people!" You don't refer to people you have known personally for many many years as "you!". It really is intuitively obvious, slayer killed Asmodean.
A little learning is a dangerous thing.
