My biggest one was the end of EOTW. When Rand kills Aginor and then is suddenly somewhere else flying around and then is walking up some weird stairway in the air and fries Ishy and saves his ghost mom from fades. I honestly felt like it was some of the worst story telling I had ever read. I had no idea what was going on. I almost refused to read TGH based on the end of EOTW.
I'm glad I started at TSR because EOTW was horrible. I don't even remember that section - the book was unmemorable.
I know many fans are enamored with Ishamael, but dying in Book II is pretty hard to recover your former level of threat. RJ basically undermined the Forsaken by showing the one that, next to Lanfear, is supposed to be the most powerful get defeated in the second book.
I have to believe that they actually do have protective wards around them, because many of the Aes Sedai seem to stroll around on a battlefield without getting struck by flying arrows, crossbow bolts or glancing blows from melee weaponry. Moiraine herself mentions that she can erect a defensive weave around the group in EOTW, but fears that the Forsaken would tear through it easily and find their hiding place, so I think it is safe to assume that all Aes Sedai can do this for themselves or their warders, and only the strongest can do it over entire areas.

My next biggest one was Ishy's flying sword fight with Rand at the end of TGH. Why on Earth does Ishy do this? He could kill Rand at his leisure with Saidin if wants, or he can kill him with even more ease using the TP. Instead he does the one thing at which Rand is almost certainly more deadly than him? He sword fights him?! WTF? This sequence still makes no sense to me. I have heard all the explanations for this and none of them satisfies me. It simply does not fit well with the rest of the series IMO.

A series of minor ones: Channelers should have protective wards around themselves much more often than they do. I have a running "wtf?" reaction to channelers walking into dangerous situations without giving themselves some basic protection.
