I have two separate hypotheses to disclose. Please judge them on their own merits.
First, I believe this prophecy relates to Rand using Callandor with his two channeling girlfriends. Rand, Elayne, and Aviendha being linked through the Warder bond may not be enough to consider them one, but I think it will be a key factor in helping this along. In other words, through the combination of the link through Callandor, the Warder bond, and the love they share for each other, they will have a unique opportunity with regards to controlling the One Power due to their unity in spirit.

Everyone is assuming that the prophecy refers to Callandor. But it just occurred to me that we have heard of a "Blade of light" before that. More specifically, Rand refers to whatever weave or technique he used to sever Ishamael from the DO in EotW (the dark wires) as a blade of light. He again uses it against Asmodean and it is called a sword of light.
Is it possible that everyone is wrong and the blade of light in this prophecy is not Callandor at all but rather the power that Rand has to disconnect the forsaken from the DO. Maybe he will use it against Moridin at the end while the two other ta'veren, Mat and Perrin will stand by his side.
I think the point of adding this part to the book so close to the end was to foreshadow Rand's transformation on top of Dragonmount. "He shall hold a blade of light in his hands, and the three shall be one." When Rand runs away and ends up on Dragonmount, he has three "personalities"- Cuendillar Rand, Lews Therin, and "the quiet voice in his heart". I think the blade of light refers to the one power (or the access key), and his transformation leads to the three becoming one.
