I thought the skin became tighter because the neurological damage made them literally uptight and less relaxed. We see the damage every time someone swears on it - it's not a pleasant experience.
but we see those physical changes. The tighter skin on the face and the shortened life span. Those two things imply the physical aspect of the oath taking.
