In TGS Bair and Sorilea examine Semirhage, and they remark on what one of the Shadowsouled looks like. They circle her and voice their opinions on how she looks and speaks.
It's strange that Bair didn't recognise Semirhage. Earlier in the series, Amys says that they have seen the Forsaken walking Tel'aran'rhiod, including some of the males. While it's possible that Amys and friends only saw a few of the Forsaken, Birgitte says that all of them have been stalking one another in the World of Dreams, except for Aginor and Balthamel (who were dead).
I would have expected Bair to recognise Semirhage.

They only saw Semirhage from afar, if they actually saw her (Bair's reaction indicates that she simply didn't see her).
I think it's rather dubious, that no one among Rand's group saw Semirhage at the Cleansing, although RJ stated she was there. Apparently the incompetent Semirhage was there, but didn't do anything..
