I think this is pretty well believed, but I want to go on firm record for thinking it now before it's revealed in later books.
When Moiraine and Lanfear tumbled through the the ter'angreal doorway that melted, they were burned out or stilled (I'm not sure it really matters which). This explains why Lan's bond was cut. Moiraine is still captive there and will be rescued by Tom, Mat and Noel in ToM. Moiraine will likely be healed by Finn at some point. Otherwise, she'll live a happy rest of her life with Tom as a non-channeler.
Lanfear was killed. She was either executed or she committed suicide. The DO transmigrated her into Cyndane and ordered her healed. I believe she was probably healed by a BA yellow from the rebels but it's also possible that a female forsaken did it. We know it wasn't Graendal. It could have been either Moghedien, Mesaana or Semirhage. I think Moghedien is the most likely of the forsaken since she could have easily picked up the healing weave in Salidar and she is bound to Moridin, but the timeline needs to be checked on this. This is why Cyndane is lower in strength than Lanfear.
This is the most simple and reasonable explanation for things as we know them now. IMO, other theories are unecessarily complicated, and therefore not very likely.
