We learned from Min's PoV that the sword was power-wrought-- which means it was made in the Age of Legends (or perhaps an earlier turning of the wheel).
It makes most since that it would be LTTs sword, but it could also be one his top generals or hundred companions. Who knows? But if Hawkwing and Rand both had, it would make sense that it was also LTTs.

Could the sword have been made by Ishamael or Dreadlords at a later date? Maybe even as some sort of dark ter'angreal. Probably just a coincidence but Rand does seem to get a lot darker in mood after he gets the sword.
