My own thoughts:
1. Demandred is a Seanchan general. I arrived at this unscientific conclusion based on the fact that he, Mesaana and Semirhage had a plot together, according to Graendal. Semirhage used to be in Seanchan, Mesaana was in the Tower. Despite Semirhage having been captured and balefired, the Black Ajah STILL knew of the HIGHLY SECRET Seanchan raid ahead of time and managed to escape. This points to Mesaana's surviving co-conspirator, Demandred, being in Seanchan. Furthermore, Demandred claimed to have an army (and liked commanding them), but Graendal (discounting the Seanchan for some reason) couldn't think of any armies big enough left in the world other than the Borderlanders.
The BA didn't flee the Tower until after the raid. In fact they didn't leave until after Egwene purged the rebels. It seems pretty obvious that Mesaana, or one of the BA like Alviarin who knew everyone, found out that the rebels had found and executed most of the BA in their camp. Also, it seems highly unlikely that the Shadow would have two Forsaken managing one group of people, especially given how high up Semi was placed, there'd be no need for another Forsaken with the Seanchan. Other possibilities would include Murandy which was just united with help from the Band.
It fits that he is commanding armies of Seanchan.
2. Asmodean was killed by Lanfear. She might not even have been Cyndane yet when she did it. She had just fallen into the realm of the Eelfinn and was granted three wishes if she agreed to pay the price. She probably agreed, with one of the wishes being to kill Asmodean, at which point she probably was given temporary access to Caemlyn to kill him.
According to RJ the Eelfinn can't affect things in the real world, there's no way she got out and was then pulled back in. Therefore there's no way Lanfear is the killer since she was held, and presumably killed, by the Eelfinn. Also, given the Finn's feelings about the Shadow it seems unlikely that they would give her any wishes, regardless of the price.
That's not a new position, but I do agree that Graendal's POV seemed to indicate she hadn't really done much for the Shadow and if she had killed Asmodean, she would have thought about it.
I don't see why she would think about Asmo at any point in her POV in tGS.
Human beings, who are almost unique in having the ability to learn from the experience of others, are also remarkable for their apparent disinclination to do so.
