Here are the Forsaken as they stand...
1. Aginor died in EoTW and was reincarnated later as Osan'gar. He most likely died during the Cleansing of saidin, though this is not definitive, but more than likely. Considering he was burned to death, he could be reincarnated again if the Dark One is willing to give him a third chance for the Last Battle, which is might or might not do.
2. Lanfear is running around as Cyndane.
3. Moghedien has survived and is running around recruiting for the Shadow and serving Moridin.
4. Demandred has survived and is somewhere rallying a force of some kind. We know less about him than any other Forsaken.
5. Ishamael was killed in TDR by Rand and has been reincarnated as Moridin, who is now the Nae'blis. He is alive and well and the leader of the Shadow.
6. Be'lal is dead permanently. Moridin said that balefire is the final death for the Forsaken and he was killed with balefire by Moiraine.
7. Rahvin is also dead permanently. He was balefired by Rand.
8. Balthamel was killed in EoTW by Someshta, and was reincarnated as Aran'gar. She escaped Salidar with Delana and is presumed well, although she made no appearance in the new book.
9. Graendal is probably dead, but this is an area of debate. We only saw her in the Prologue. Rand believed she was in a fortress and he balefired the place using the Choedan Kal. The rapid reversal of Compulsion on her victim suggests that she was indeed killed, but it will be difficult to say. She's a notorious trickster and may have survived.
10. Asmodean is dead, killed by an unknown assassin. Considering his betrayal it is unlikely he will be resurrected for the Shadow. His almost instant death also suggests balefire, but this is by no means proven.
11. Mesaana is alive and well and in the White Tower disguised as someone.
12. Semirhage is dead. After escaping her captivity, she collared Rand with a domination band, but he escaped using the True Power and used it to balefire her. Her death is permanent.
13. Sammael is dead. Mashadar consumed him. It reportedly consumes the soul so the finality of his death is debatable. Mashadar may devour your thread forever, or merely kill you. It may be even more absolute than balefire, or less so - we don't know. If the Dark One can resurrect him, he might.
