She had absolutely nothing to do with it.
I would not call it her FAULT, per se (she should have better guards than a bunch of fellow adolescent nobles pretending to be nobles, but also had no reason to expect a Shaido attack.) That was, however, the ultimate cause of Perrin and Co. getting trapped by the dreamspike (it was the basis of the plan to so trap them, in fact,) which makes Faile the ultimate cause.
