As a result, during Perrin's plotline, Tam was there with him, but when we jumped to Rand's plotline, which took place later than Perrin's, there is Tam. In Rand's timeline, Tam came to Tear at the end of tGS. Perrin's timeline in ToM was concurrent to Rand's from tGS, so Tam left him to go to Rand near the end of Perrin's story in ToM, but all of Rand's story in that book took place later. Thus, you would go from Ghealdan and Altara with Perrin featuring Tam, to Tear with Rand, also featuring Tam.
