In the Epilogue, Shaidar Haran confronts Graendal regarding her failures. He credits her with causing the death of three of the Forsaken. He blames her for Mesaana’s death. The second is obviously Aran’gar but who is the third? Asmodean?

Yes. It's pretty much confirmed in Q&As before the event and in the glossary so this Whodunnit is solved.

Graendal then begs to be given another opportunity to defeat Rand as he thinks that she is dead. Shaidar Haran informs her that that task has been given to another. Is the consensus that this is Lanfear/Cyndane who appears in Rand’s dream at the end of the Epilogue? Lanfear did boast before that she could enter Rand’s dreams whether they were warded or not. Moridin also has the link with Rand. Is he tempting Rand by appealing to both his instincts to protect women and also baser human feelings like lust? Rand “feels hatred, concern, and-like a seething viper within him-desire.” Reminds me of the temptations of Jesus Christ in the desert. Thoughts?

Not sure about the consensus, but I think that was Lanfear/Cyndane who appeared in BuddhaRand's dream. I don't think Moridin is related to it, even though he may be aware that Lanfear/Cyndane was given another chance ('Cyndane' means 'Last Chance' in the Old Tongue, so this is somewhat poetic for the DO to do). Moridin is a failure in terms of tempting Rand - I think he doesn't have the understanding of human nature to really appeal to these concerns. So the modus operandi fits Lanfear/Cyndane to a 't' - especially since this is what she did earlier in the series as Selene.

But Cyndane is mindtrapped by Moridin. Surely she cannot execute any action against Rand without Moridin's approval or perhaps at his direction?
