It is built around unrealistic circumstances and coincidences. It's the reason anybody actually reads it.
One your examples is a bad one, there's no way Faile's death wouldn't have been hugely significant, unless Perrin died for completely unrelated reasons without learning of it or shortly thereafter.
Gawyn dying to the first Bloodknife, however, would have been suitably gratuitous and narratively pointless to qualify as bad writing. Dying to the second attack, on the other hand, would have been fine.
