I think she would understand some of the health implications.
And although Aes Sedai normally shouldn't have any trouble whatsoever dealing with little things like cholera, it's different if there is not enough food (which was the case, with all the food rotting away), since the Healing won't work as well.
You don't need any special training to see that garbage in the street will kill people after a few weeks. After a major disaster in our own world the first thing that sets in is cholera.
Also, it's not like the whole White Tower is making a drive to clean the sewers. Like most things, Egwene noticed it in passing and decided to fix it but it's been heralded as her sole driving force.
Elayne deals with it in Caemlyn ... it's specifically called out by Norry that she must see to the city sewers because of rat infestations.
And even Elaida called it out in KOD
