Would you unleash the Balefire in my service, Demandred?

Has anybody got any theories of where/when Demandred may have used Balefire? I think that somewhere in the story there might be a telltale "blooper" where something is left unfinished or doesn't seem quite right. There's a good chance he was behind Masema (and I think he was behind Asunawa as well), but neither of these necessitate the use of BF.
There's not much info to go on, but here's what I have:
- It's something that only Demandred can do, since he is the only FS who we know was explicitly asked to use it. Why would he be asked over anyone else?
- We also know that it was very important. The DO doesn't like Balefire, but he asks Demandred to use it anyway. There was some need to reach back in time and undo something.
- This would likely have occurred around the time of TFoH, LoC and ACoS given the time frame of Demandred's orders.
- Demandred knows exactly how dangerous this task could be - despite being in the very heart of Shayol Ghul, he still hesitates for a moment after hearing the order.
- So much so is Demandred afraid of using BF that he thinks to himself that killing Rand will mean he would no longer need to use it himself. Note that he doesn't consider using BF to kill Rand.
- He then is told "Who will die and who will live." So murder may be part of this.
- No, he didn't go back and save/kill Asmodean. This order came after that, and the DO clearly did not want Asmo alive.
This might be the "little detail" which Brandon mentioned would have a significant effect later in the story.
