All of those things can temporarily interfere with your souls' ability to be returned to the soul reservoir, but I don't think anything can permanently destroy the soul until the Dark One truly breaks into the Pattern and has full reign.
IMHO
1. Being kissed by a draghkar = This is only a temporary biological housing for the soul; it probably returns to the soul pool once the draghkar dies or is killed. Having more souls probably increases a draghkar's crooning ability making it more powerful
2. Being used as a vassal to forge a Myrddraal blade = Once again, this is only a vessel, and the soul returns to the pool of souls once the sword is broken
3. If your mindtrap is destroyed = I think your soul is only temporarily housed in a mindtrap, and it will return to the pool of souls
4. If the DO wants to destroy your soul (assuming you're in the Pit of Doom) = I don't think the DO can do that...yet...because It is still locked outside of Creation, I doubt that it can do anymore than hold a soul temporarily
5. Death by mashadar = Your soul is captured, and slowly returns to the Wheel as it is "digested" by Mashadar
