Short version:
Running Around = 3. Berylla Naron (Bl), Jeaine Caide (Gr), & Rianna Andomeran (Wh) were all assigned unknown tasks by Moggy and haven't been heard from since.
Captured = 6. Liandrin (R) w/ Seanchan. Chesmal Emry (Y), Eldrith Jhondar (Br), Temaile Kinderode (Grey), Falion Bhoda (Wh), and Marillin Gemalphin (Br) are all Elayne's prisoners in Caemlyn.
Dead = 4. Amico Nagoyin (Y), Joiya Byir (Grey), Ispan Shefar (Bl), Asne Zeramene (Green).
Long Version Here's the 13.
1. Liandrin (Red). Shielded and with the Seanchan.
Ordered by Moggy to hunt for Nynaeve, but tried to Compel her was was shielded and left to be a servant to Darkfriends there. Was found by the Seanchan and made Suroth's da'covale.
Could still be one, or possible a damane since Suroth decided to have her collared and held by a DF sul'dam at one point.
2. Amico Nagoyin (Yellow). Dead.
Guarded the Wondergirls in Tear, was stilled by Egwene and captured during Wondergirls escape. Killed by Slayer in Tear for divulging information (Tanchico). Death in tSR, revealed to be Slayer in WH.
3. Joiya Byir (White, then renamed as Grey). Dead.
Captured the Wondergirls in Tear. Beat Egwene savagely. Was shielded by Ewgene in T'A'R and subsequently captured. Killed by Slayer in Tear for failure and divulging info (plot to free Taim and set him up as evil false dragon so world wouldn't unite behind Rand). Death in tSR, revealed as Slayer in WH.
4. Ispan Shefar (Blue). Dead.
Was trying to find the cache of items of the power in Ebou Dar (for Moggy) and captured in the battle for the cache. Was poisoned (along with Adeleas)while being questioned as Elayne and the Kin rode toward Andor in tPoD. Her killer was revealed as fellow Black Sister Careane Fransi in KoD. Careane is killed by Vandene when she is revealed as Black by Marillin Gemalphin and Elayne is captured.
5. Asne Zeranene (Green). Dead.
Ordered by Moggy to find Nynaeve. Stole the balefire rod from her and used it outside Caemlyn.
In the House on Full Moon Street confrontation that claimed Ispan, Vandene, and Careane. Falion, Marillin, and Lady Shiaine are cpatured by Elayne, Vandene, Sareitha, and Careane. Asne and 3 more of the Original 13 (Temaile, Eldrith, Chesmal) return and surprise them. Asne stuns Elayne & Co with the black rod, Careane is revealed to be Black by Marillin, Vandene kills/stabs Careane, then Chesmal kills Sareitha and Vandene. Asne is killed by the Windfinders lightning strike as they are bundling Elayne away.
Her four Warders, and the 4 other original 13 Blacks with her (Chesmal, Eldrith, Temaile, and Falion).
6. Chesmal Emry (Yellow). Elayne's prisoner in Caemlyn.
Participated in putting Tamra to the question and her murder in NS. And announced Meilyn's death as natural (NS). Bragged about inducing Reds to kill Sierin, who was about to order her arrest.
Ordered by Moggy to hunt for Nynaeve. Killed Vandene and Sareitha. Captured by the Windfinders after the lightning strike. Elayne's prisoner (that Reds killed Sierin could bring down some folk in the Tower, yes?).
7. Eldrith Jhondar (Brown). Elayne's prisoner in Caemlyn.
Ordered by Moggy to get Nynaeve. Captured in the battle outside Caemlyn after the confrontation on the House on Full Moon Street.
8. Temaile Kinderode (Grey). Elayne's prisoner in Caemlyn.
Ordered to Hunt for Nynaeve. Captured in battle outside Caemlyn after confrontation on Full Moon St. Earlier had tortured Amathera, broke Liandrin, and spied on the girls in T'A'R.
9. Falion Bhoda (Wh). Elyayne's prisoner in Caemlyn.
Ordered to find ter'angreal cache in Ebou Dar with Ispan. Planned to capture Nynaeve for Moggy and took a bunch of men with her to the Rahad. Was shielded and given to Shiaine as punishment for her failure. Was with Shiaine on Full Moon St. and later captured.

Sent to Shiaine by Moggy. Contacted Careane to get her to kill the Kin with the power to spread fear. On Full Moon St. with Shiaine and subsequently captured.
