The short answer is "we don't know". the long answer is "hopefully we'll find out something in the prologue". The longer answers are:
It probably was discovered while digging the fortifications around the house
It's not clear whether it's just the scabbard or if it's both sword and scabbard. Despite general consensus I think it's both.
Some think it might be Guaire Amalasan's sword/scabbard.
Recogintion: many and varied theories so far (and if not solved in the prologue more to come)
1)recognised from the Heros of the Horn in tGH
2)recognised on his way thru the Rhuidean glass columns in TSR
3)bleed thu from Mat's memories via the 'wash of colors' pattern thingie
4)recognised from the portal stone "flashbacks" in tGH

6)recognised in a book description or illustration during his reasearch at Tear and elsewhere. (my personal favorite)
7)probably a hundred others I haven't seen yet.
