Scorpius/Harvey/John on Farscape.
Scorpius wanted to get information from John, so he implanted a Neural Clone of himself insie John's head (essentially a computer chip with Scorpius's personality.) Once it was done, Scorpius removed it. However, it left an imprint of itself.
Enter Harvey, the left over imprint still inside John's head. It would talk to him and help him out occassionally. But Scorpius was still out and about.
Same situation. Sylar was always still there. Matt just imprinted Nathan over Sylar's personality. When he did this, there was some kind of 'bleed' and Sylar's personality somehow imprinted on Matt. So he now has a 'Head Sylar'. The real Sylar was set free when the 'Nathan' personality was set free.

Agreed. I wish that they would just do something useful with Sylar or kill him off finally.
