Alien was the first movie to scare the shit out of me. I never have been the sort to lose sleep, but one night when I was about 8, I did NOT sleep.
Also, I thought that Event Horizon was scary. Terminator scared the bejesus out of me as a kid.
Blair Witch was scary, but only once. Having roamed the woods at will when I was a kid, I could identify with the whole "it's creepy out here in the quiet forest" feel of it.
I'm hard to scare, really. Movies like "28 Days Later" are scariest for me because honestly, that crap could happen TODAY. Who knows what crazy little germs some nutty scientist is cooking up in a secret lab somewhere? And OMG, the scene when they're in the tunnel and have the flat tire, then here come all the rats.............eeeeeeeeeeeeeeeeeeeeeeeeeeeee!!!!! OMG RUN RUN RUN!!!
