Carnage brought a whole different kind of evil to Spidey's world that is not suitable for kids to watch. Firstly, the serial killer Cletus Cassidy was introduced when Peter was well into adulthood, with adult problems and he was already married to MJ. Secondly, he is extremely psychotic; like Joker times 10 without any humor. Finally, There is no real way to introduce Carnage into the movie without establishing Venom as a real character first. What, the movie starts with a barely even seen Venom giving birth to red goo that somehow slips onto Cassidy? Don't think so.

In all seriousness though, he was based off the Joker and it does show. And I don't actually find him all that more depraved or disturbing than the most recent portrayal of the Joker, actually. They can always tone down his body count to make him a bit more palatable.
As for the rest, he doesn't need to be introduced exactly the same way he was in the comics. A short prologue showing "infamous serial killer Cletus Kasady" finally being arrested and thrown in jail would be enough to establish the character. Besides, by the third movie, Spidey was already an adult and considering getting married. Anyway, I'm not sure Spidey's age and marital status really have all the much affect on whether or not Carnage would work as a character.
Yeah, the Venom thing would be a problem, since they killed him off and all, but there was that little piece of the symbiote left over at the lab, and they could have used that in the future to introduce Carnage. I'm not saying I would have definitely had to see him in the hypothetical fourth film, but as long as they kept going with the already established universe/timeline, I was hoping I might get to see him eventually. Even if he had to be toned down or his origin story tweaked a bit (or even a lot). A crappy version of Carnage is better than no Carnage.
