Crack in the Tardis? - Edit 1

In the eleventh hour-episode, at the very end, the Doctor walks past a screen in the Tardis, while trying to convince Amy Pond to come along. The screen shows a wave form, and underlying it is the shape of one of the cracks.
The Doctor turns the screen off, and the whole image, everything we see, flickers, and the Doctor leans against the screen for a second before shrugging it off. Something significant seems to happen here, as though the universe changes, or splits. Into to separate timelines where things happen differently? Any ideas?
A little while later, he says to Amy that he's definitely a mad man with a box. Is this a sign that he'll be locked in a box (Pandorica) for a long time and go mad?
Also: Who built the time machine in the Lodger? The Doctor calims someone has tried to build a Tardis, but who? And why have they come to Earth?
