For the same reason I can collect Social Security if I wanted to and you can't. Because I have been paying into both since December of 1968 when I got my first job and am now past the age where I qualify. Whereas you are not.

A public option that allows younger people to buy into Medicare would require them to pay premiums. Less than typical private insurance, because Medicare had much greater bargaining power, and being a government run program, wouldn't hoard the savings but pass it on to citizens who buy in.
