I mean, it's really only an issue if you're repeating it for royal blood issues or because a smallish group of people have isolated themselves from the rest of the world geographically or for something like religious reasons.
Even brother-sister incest isn't that big of a deal until a couple of generations goes by. Icky, but not 'deformed babies' icky.
Hollywood has the best moral compass, because it has compassion.
