These are the changes that I have come across in discussions with friends. (spoilers)

At the start, you choose Ashley, Kaiden, or "Lots of casualties. That is it. Only choice other than background war hero, solo survivor, etc from the other games.
In game, there are a bunch.
No Drell from 2 (forgot name)in the hospital, so none of the scenes involving him show up. No Wrex or Grunt in Krogan story line. Werx's less nice brother Wreave is the leader of the Urdnots. Eve dies because you didn't get the cure research data that Moridin's apprentice was working on. No Jack at the school. Legion makes an appearance, but his name is Geth VI and he doesn't recognize you.
Essentially, Shephard did none of the side missions in 1 or 2, so any character or choice you didn't HAVE to get or make, are not showing up. I am hoping that they might be on the second playthrough, but it is a tiny hope.
