If you have built up piles of Cholesterol, heavy exercise could dislodge some "plaques" or such. They're not causing problems for you now, perhaps, but if they're free floating and get to an important capillary bed? You could be in a world of hurt. They could clog or burst a vessel in your brain, lung or heart and you'd be in a bad place.
