I was covered for countless blood work and x-rays, but had to have prior approval for MRI and chiropractic care. BCBS covers my general health insurance but United Health care covers hospital and lab stuff. It sounds confusing but the office staff always bills the right place.
I think someone has already told you to not go to a hospital, and to find a general practitioner who can refer you to people who can order tests. In most cases, the medical office staff will deal with the insurance companies. Sometimes they even call to schedule specialists and lab appointments, but sometimes you have to get prior approval on your own. If you ask, they should be able to do it for you if they don't offer. I think insurance companies are generally more likely to approve if the doctor's office calls, they know what to say to get the tests approved.
I'm wondering if you go to your insurance company's website (or call an 800 #) and type in your insurance card number if they will give you any information on the type of plan you have? The number might even be on the back of the card... The website will even help you with a participating provider list.
If your parents won't give you the card for whatever the reason, contact your therapist's office (or some other place where you're already a patient) and say you've misplaced your policy/card number. They may give it to you.
Good luck.

I believe all news and research that supports my opinion, and dismiss the rest as conspiracy and lies.
