1. Full-on socialized medicine in the European model (or the Canadian one). This would by necessity require tort reform as well, as well as price controls on certain procedures and equipment. Private clinics and doctors could operate outside the system for those who can pay for better service, just as with everything else in life.

2. A fully privatized system with no Medicare or Medicaid. Government vouchers would allow for graded assistance in buying insurance to help lower-income individuals, and insurance rates would be set by the government, just as utility company rates currently are. Lower rates would make insurance a reasonable proposition for more people. If hospitals were allowed to refuse people without insurance or payment for all but the most emergency procedures, it would act as a massive incentive to have insurance. If you don't get insurance when it's easy and cheap (and in some cases, subsidized) go find a dark corner and die.
