Also, even where it is mandatory it's to have auto insurance specifically to drive a vehicle on a public road. You aren't required to have it.

Well, being required to have it to drive on a public road and being required to have it period are basically the same thing as far as my meaning went. But I didn't know that some states don't require it. Do you know how many, roughly?

These days very few, or two, Wisconsin and New Hampshire, there's no federal law on it and states laws very a lot both in nature and time of enactment. Some places have or had laws where you could simply place a bond or prove you had means and assets in excess of what insurance would cover anyway on the theory that it's really silly to make a multi-millionaire have liability insurance, essentially forcing him to pay a statistically higher amount of oney when he could just as easily take, say, $20,000 and dump it in some interest paying bond.

What happens when an uninsured driver in a state that doesn't require insurance causes an accident that injures someone? Does the injured party simply have to sue the uninsured driver on their own? What if the uninsured driver has no money?

Same thing that happens if a person comes up and smashes your car with a baseball bat - or accidentally with a shopping cart - you can sue them and if they have no money you're semi-screwed.
