Nico is the shadow fallen, I assume it was him. Or it could be lasciel, who wanted him to fall to the dark side (knight of winter) and so get a thirst for more power resulting in him calling on her next.
I say Lasciel because the fallen are at essence - parasites. They need a body to live in and influence the world through. I assumed it was a parasite of Harrys, rather than a parasite in general. If we go vampire (which could be counted a parasite in general), then i'd put money on Lara Raith, she certainly has teh connections. But I just don't think Mab and Demonreach would let Lara in on things, or that Lara could keep it all so quiet. So I'm sticking with a Lasciel or one of her shadows (not the shadow lash who is probably dead) who was able to keep the heart beating whilst Harry was - absent.
I was honestly expecting Lasciel to be the one who whispered in his ear, leading to his death. I don't think that now, but I did for a minute when I read the scene.
