The Mad King had half a dozen non-Targ Hands, at least and i can't remember many example of targaryens filling the post.
Maegor the Cruel was Hand of the King to his half-brother, Aenys.
Viserys I was only king for 1 year, but for the previous 15 he had served as Hand to first his nephew Daeron I, and then Baelor the Blessed.
Daeron II's Hand was his son and heir, Baelor the Breakspear (who appears in THE HEDGE KNIGHT).
Aerys I's Hand was his half-uncle, Brynden 'Bloodraven' Rivers.
