It's never really jarring, because obviously actors are supposed to be in multiple films, and I never really forget I'm watching a film anyway.
If a character in a book has the same name as another character from a different book, that can throw me off. I picture the wrong character. Off the top of my head, I kind of laughed comparing Denna from The Name of the Wind to the Mord-Sith Denna from Sword of Truth.
Oh, and now that I think about it, Sarene from Elantris was somewhat morphed with the White Ajah Aes Sedai Sarene from The Wheel of Time. They still look a little bit alike in my head.
